Curious to know who killed Kat in Backrooms?
Lukita Maxwell’s character dies shortly after her boyfriend, Bobby, meets a similar fate at the hands of one of the creatures. She tries her best to rescue him but fails. Focusing on her own survival, she and Clark run through a red-lit room and, at one point, get separated. Soon, her story is cut off abruptly after she repeatedly warns, suggesting that she experienced an even more brutal end than her boyfriend did earlier.

Who killed Kat in Backrooms’ ending, explained
Although the movie doesn’t reveal who killed Kat, there are suspicions that it could have been Pirate Clark.
Kat runs through the backrooms to make a final attempt to rescue Bobby. Clark follows her; together, they sprint through festive rooms. At the time, Clark comes across two of the Backrooms’ Still Life entities. The scene then transitions to near the pool when Clark hears Kat’s voice and puts his camera down. He’s unable to see her because they are separated by a one-way window. However, Kat could clearly see him.
Things take a horrific turn when the camera is held by something unseen, as evidenced in the footage-style scene. Kat repeatedly warns Clark to turn around. The scene cuts then and there, with Clark’s story having jumped forward in time. His therapist, Mary Kline, looks for him in the backrooms.
Mary eventually finds Clark, but it is already too late; he has gone insane by then. This happens because he has been stranded there for a long time, facing the horrors and dealing with his own trauma.
Clark then opens the fridge, where Kat’s lifeless head is kept duct-taped, as Mary watches. He doesn’t explain Kat’s death but only says that he tried to save her. However, it is implied that something worse might have happened to her behind the scenes.
